Face to Face in home visit

·       $300 for an initial assessment

·       $75 for follow up (up to 15 minutes in length)

·       $150 for follow up (up to 30 minutes in length)

·       $225 for follow up (up to 45 minutes in length)

·       $300 for follow up (up to 60 minutes in length)

• Lab work will be drawn in the home and have an additional fee.

Video Telemedicine visit

·       $200 for initial assessment

·       $50 for follow up (up to 15 minutes in length)

·       $100 for follow up ( up to 30 minutes in length)

·       $150 for follow  up (up to 45 minutes in length)

·       $200 for follow up (up to 60
